BY FAX: Fax: 1-336-766-3656 Print the ORDER FORM, fill it out completely and fax. Faxes are accepted 24 hours a day.		BY EMAIL: onesource4media@triad.rr.com On line email order form. Enter your order information in the boxes, then click on the submit button. We will respond within 24 hours.

PAYMENT OPTIONS
Checks should be made payable to: OneSource4media. Purchase orders are only accepted from companies located in the U.S. and Canada, and they must include the purchase order number and the name of the authorizing agent.

WIRE TRANSFER INSTRUCTIONS FOR OVERSEAS ORDERS:
All items must be prepaid in U.S. currency in order to be shipped. You may wire transfer funds to our financial institution for an additional $25 wire transfer fee.

SHIPPING AND DELIVERY
Outside the U.S., orders will be shipped via air mail. Assuming no customs delays; allow up to four (4) weeks from our shipping date for overseas delivery. Within the U. S. and Canada, orders will be shipped via ground service unless customer requests otherwise.

15-DAY NO-RISK RETURN POLICY
We are not happy unless you are completely satisfied with every item you purchase from OneSource4media. If you are dissatisfied with an item for any reason, you may return it within 15 days with a written explanation of your dissatisfaction and OneSource4media will refund the price paid. Items returned for reasons other than damaged or incorrect shipment must be received in good condition, and in such cases, the shipping and handling charge will not be refunded.

Video/DVD course lectures are non-returnable.  Defective or damaged Video's/DVD's will be replaced at no charge.

VIDEOTAPE FORMATS - PAL Format:

Other Countries use the NTSC format for videotapes.

Current Protocol orders do not require 9% shipping and handling charges.

Please type or print clearly. Use our OneSource4media  Item Numbers when ordering and fill out the form completely, including your telephone and fax numbers. Attach a copy of your purchase order if applicable (U.S. and Canada only). Incomplete forms may delay shipping.

• Quantity discounts are available, for more than two (2) copies of the same tapes. Please contact OneSource4media.
• Academic discounts are available only to organizations that grant degrees, and with the understanding that the.videotapes are to be used exclusively in an academic environment. Please contacth OneSource4media.
• All orders from outside the United States must be prepaid in U.S. currency.
• All prices and availability are subject to change without notice.
• If you are dissatisfied with an item for any reason, you may return it within 15 days and OneSource4media will refund the price paid minus shipping and handling charges
